The Ogdensburg Pee-Wee Football Association will be holding sign-ups at Howland Pump during the hours from 9 a.m. to 4 p.m.

The registration cost is $75 per child, $45 for the second child of the same family and a maximum of $125 per family.

Nerf football players (ages 5-6) will receive a nerf or mini league football, a pee wee football jersey, six weeks of instruction and a pizza party. The fee for the Nerf level is $30 per player and play begins in the first week week of September.

I/we the parents of the above named candidate for a position on a Pee-Wee Football Association team hereby give my/our apporval to his/her participation including transportation to and from activities. I/we do hereby waive, release, absolve, indemnify the organizers, sponsors, supervisors, participants and persons transporting my/our son/ daughter, except to the extent and in the amount covered by accident liability insurance.

I/We understand that the medical insurance provided by the Pee-Wee Football Association is under an excess benefit policy and that it provides a limited coverage only when the participant is not covered under any other insurance program. i/we certify that my/our son/ daughter is physically capable of participating and has no known health related problems that could be aggravated by participation in the sport of football or any of its associated physical challenges. I/we will furnish a certified birth certificate of the above named candidate upon request of league officials.

I/We understand that all football equipment provded by the Pee Wee Football Association is loaned to the candidate to be used while participating in Pee-Wee football during the Pee Wee football season only. I/We understand that this equipment is to be returned at the end of the season. i/we further understand that team members are authorized to use this equipment during team practices, games and sanctioned events only as instructed and directed by the coaching staff.

I/We understand the importance of sportsmanship and will help to encourage our son/daughter to demonstrate good sportsmanship on and off the field. I/we agree to display good sportsmanship at all times or risk disciplinary action by league officials. i/we further agree to abide by the rules, regulations and decisions of the Pee Wee Football Association officials.
