The Ogdensburg City School District has announced the Health Office Summer Hours for 2014.Nurse(s) will be in the Health Office, on the following dates/times, to facilitate Fall Sports Procedures:

Thursday, August 7: 8 am – 1 pm. Friday, August 8: 8 am – 1 pm . Thursday, August 14: (Varsity & JV Sports Season Practices Begin): 8 am – 1 pm.

Monday, August 25: (Modified Sports Season Begins) 8 am – 1 pm .

A Sports Participation Health Permission Slip (for the 2014-2015 Fall Sports Season) may be picked up in the Health Office or High School Office. The completed form must be submitted to a Nurse, in the Health Office, on one of the above dates.

In addition required forms maybe accessed on online:

•Log on to the Ogdensburg City School District website at http://www.ogdensburgk12.org

•On the left side of the homepage, scroll down and click on “Health Services”

•On the right side of the Health Services page, under “Files & Forms”, users will need to scroll down and select the forms needed; an athlete must complete/submit two (2) of the forms listed: 1.“Athletic Health Form”

2.“Sports Participation Form”.

NOTES: A student-athlete will not be allowed to begin practicing with an OFA sports team until and unless he/she has a Permission Slip, from the School Nurse, verifying that he/she has completed everything and is cleared to begin participating on an OFA sports team.

A child that requires medication (i.e. Epi-pen or inhaler), he/she will not be allowed to practice without a current self-directed order from the athlete’s primary care physician and corresponding medication. The above referenced website prints a “self-directed order” form (pages 2 and 3 of the “Athletic Health Form”).

This form requires the following signatures: two (2) by your child’s physician, one (1) by the parent/guardian, and one (1) by the student/athlete.

Both the self-directed order and the medication must be presented to the Health Office on one of the above dates in order for the student-athlete to participate on his/her first day of practice.

**If the student-athlete requires the use of an Epi-pen, two (2) devices need to be provided to school personnel; one (1) to be stored in the Health Office and one (1) to be given to his/her respective coach.
